Insurance:
-
I am in-network with United HealthCare and Aetna.
-
Coverage for nutrition services depends on your individual plan. I will verify your benefits prior to your appointment to determine your coverage.
-
A physician referral is required for covered services and should include specific diagnoses and diagnostics codes.
-
Insurance coverage is for individual visits only and not packages.
-
-
For those with other insurance plans, I can provide a superbill for you to submit to your insurance with a claims form. Reimbursement is dependent upon your individual plan. It is the client's responsibility to determine if insurance will provide reimbursement for nutrition services, but I can assist with questions regarding procedure and diagnostics codes.
-
For those paying for services out of pocket, an electronic invoice payable by credit card will be provided after sessions. Payment in the form of credit card is preferred, but check or cash can be accepted.

Daytime and evening hours are available, both virtually and in-person.

Saturday appointments are available two days per month.

We can discuss your scheduling needs during an initial free 15-minute phone call.
